    Daycare Statement 
        A form provided by or signed by your child care provider.  Information on the statement should include the following:
            1. Client/Taxpayer Name
            2. Child Care provider name
            3. Child Care provider Employer Identification Number or Social Security Number
            4. Child Care provider’s address and phone number
            5. Name of the child/children
            6. Total amount paid for child care for the year
